Provider Renewal Application

You may access the Provider Renewal Application with the username and password used to access the Education Provider Online Reporting application. If you do not already have a username and password, you must create and register this information. Select the Sign Up link on the login page to complete this process. You will need your provider approval number and provider index number. If you do not have your provider index number, please contact to obtain it.


Each page of the application will contain detailed instructions on how to complete the application. You may obtain additional information by clicking on the ? symbol.

All instructors you select to renew must complete a separate Instructor Renewal Application. Once selected on the Approved Instructor List page of the Provider Renewal application, a renewal notice will be sent to the instructor email address in our records. The status of the instructor will remain pending on the Provider Renewal Application until the Instructor Renewal Application is completed. Once the status of all selected instructors shows Complete, you may complete the Provider Renewal Application.

Please Note: If you have no currently approved courses, you cannot renew online. You must login, download a paper application and mail to the address on the application with a check for the required fees.


Renewal fees are:

Renewal fees may be paid by credit card or by paper check. Electronic checks are not available. If paying by credit card your renewal will be processed immediately. If paying by check your renewal will be processed when payment is received. You may submit one check, bank check or money order in the amount of the total fee. Please attach the check to the payment invoice and mail to the address provided.

Checks must be made payable to the Superintendent of Financial Services. Pursuant to New York State Insurance Law, Section 2132(i) (3), all fees are non-refundable.

Providing a valid email address for the primary designated person, allows us to send your approval documents within 5 business days of the renewal processing date.


The Provider Renewal Application is a secured application available on the DFS portal.
