Address Change Request

An Address Change Request allows a licensee to securely log in and update all address information including the email address. Updates made will immediately be reflected on the licensee’s licensing record.  

Individuals/TBAs - the residence address needs only to be updated once.  Business and mailing addresses will need to be changed on each current class of license held.

Non-residents (licensees whose home state is not New York) will be required to certify that they are currently licensed and in good standing in their home state.

Please read all instructions carefully as all changes made will become an official part of the licensee’s record.  Additional instructions are available by clicking on HELP which appears on each page of the Address Change application.

At this time the online Address Change Request is not available to Service Contract Registrants, Life Settlement Providers, or Life Settlement Intermediaries.

  • This application may be unavailable daily from 7:30am to 7:45am for regularly scheduled maintenance.
