An Article 44 Health Maintenance Organization may not change subscriber rates more than + 10% without the prior approval of the Insurance Department.
As an aid to the public, and at the suggestion of the Insurance Department, the Plan is making available to the public a summary of facts supporting its application. A copy may be obtained from the Plans office or from the Insurance Department.

Those wishing to speak at the hearing should notify the Insurance Departments Public Affairs and Research Bureau at (212) 480-5262. In accordance with the Americans with Disabilities Act, the Insurance Department will provide a reasonable accommodation, such as a sign language interpreter, a qualified reader, etc., at no charge to any covered individual wishing to attend or testify at the hearing when such a request is made at least 7 days before the hearing.
Those unable to attend the hearing may submit written statements for inclusion in the official record within ten days of the hearing date to the Office of Public Affairs and Research at the Insurance Departments New York City Office.
