Portal Applications & Accounts
For access to SECURED applications on the DFS Portal, you must have a Portal account. You will also find a list of all of the DFS Secured and Guest (unsecured) applications available when you log into the DFS Portal:
In order to access the Insurance Bureau Frauds Annual SIU Report Online eForms, your company must:
- Go to the Portal.
- If you do not have a current Portal account, create a new Portal Account by selecting a unique member name and user ID, and entering all of the required account information, including a valid company email address. A system generated account password will be sent to the email address entered.
- Sign-up for the Frauds SIU application from the list of Available Applications
- Request access to the Frauds SIU Application. You will be notified by email of acceptance or reason(s) for rejection.

On the Portal Welcome Page select Create Account:
You will then receive a system generated account password via email. With the ID and password, you can login to sign-up for the Frauds SIU application by selecting Frauds SIU from the list of Available Applications. Click Request Access.
On the Request Access screen, type the first few letters of your lead company name, and select it from the drop down list of Insurers/NAIC. Click Submit Request to add the company to your request list.
You may select more than one company if you are authorized to file separate Frauds Annual SIU Report on each of the selected company's behalf (see below).
Wait for e-mails from the Portal Administrator and/or DFS for further instructions where indicated. Your application will be reviewed, and if approval is granted, the next time you login, you will have access to use the Frauds SIU Annual Report Application.
