Bureau Frauds Annual SIU Report
Create Account & Application Sign-Up Instructions
In order to access the Insurance Bureau Frauds Annual SIU Report Online e-Forms, your company must:

On the New Portals Welcome Page (https://myportal.ins.state.ny.us/), select Create Account.
You can view the "Create Account" instructions on: (https://myportal.dfs.ny.gov/create-account-instructions)
After the Create Account form had been completed, you should be receiving a system generated account password via email. With your valid ID and password, you can login to sign-up for the Frauds Annual Report e-Form application by selecting "Frauds SIU" from the list of "Available Applications" (see screen below), click "Request Access" link.
On the Frauds SIU Application Request Access screen, type in the first few letters of your company name, and select from the drop down list of Insurers/NAIC number the lead company for your company's Frauds SIU Annual Report information. Click the "Submit Request" button to add the company to your request list.
You may select more than one company if you are authorized to file separate Frauds Annual SIU Report on each of the selected company's behalf (see below).
Kindly wait for e-mails from the Portal Admin and/or Insurance Frauds Bureau for further instructions. Your application will be reviewed, and if approval is granted, next time you login, you would have access to use the Frauds SIU Annual Report Application.
