Insurance Bureau Frauds Annual SIU Report
Create Account & Application Sign-Up Instructions

 

Overview

In order to access the Insurance Bureau Frauds Annual SIU Report Online e-Forms, your company must:

  1. Click https://myportal.ins.state.ny.us/ to go to Portal Login.

  2. If you do not have a current Portal account, you must come to the new Department Portal site to create a new Portal Account (this is done by selecting a unique member name / user ID, and entering all of the required account information, including a valid company email address. A system generated account password will be sent to the email address entered once the "Create Account" form had been completed)

  3. Sign-up for the Frauds SIU application from the list of available "Authorized Services"

  4. Your request to access the Frauds SIU Annual Report Online e-Forms will be forwarded to the Department of Financial Services' Insurance Frauds Bureau for review. You will be notified by email of acceptance or reason(s) for rejection.

On the New Portal’s Welcome Page (https://myportal.ins.state.ny.us/), select “Create Account”.

After the Create Account form had been completed, you should be receiving a system generated account password via email.  With your valid ID and password, you can login to sign-up for the Frauds Annual Report e-Form application by selecting "Frauds SIU" from the list of "Available Applications" (see screen below), click "Request Access" link.

 

On the Frauds SIU Application Request Access screen, type in the first few letters of your company name, and select from the drop down list of Insurers/NAIC number the lead company for your company's Frauds SIU Annual Report information. Click the "Submit Request" button to add the company to your request list.

 

You may select more than one company if you are authorized to file separate Frauds Annual SIU Report on each of the selected company's behalf (see below).

Kindly wait for e-mails from the Portal Admin and/or Insurance Frauds Bureau for further instructions. Your application will be reviewed, and if approval is granted, next time you login, you would have access to use the Frauds SIU Annual Report Application.
