Address Change Request

An Address Change Request allows a licensee to securely log in and update all address information including the email address. Updates made will immediately be reflected on the licensee’s licensing record.  

Individuals/TBAs - the residence address needs only to be updated once.  Business and mailing addresses will need to be changed on each current class of license held.

Non-residents (licensees whose home state is not New York) will be required to certify that they are currently licensed and in good standing in their home state.

Read instructions carefully as all changes made will become an official part of the licensee’s record.  Additional instructions are available by clicking on HELP which appears on each page of the Address Change application.

Address Change Request is not currently available to Service Contract Registrants, Life Settlement Providers, or Life Settlement Intermediaries.

    • Use a supported browser: The latest version plus one previous of IE, FF, Chrome and Safari, are supported.
    • Disable any “Pop-up Blocker” setting in your browser before using portal applications.
    • Maintenance: This application may be unavailable daily from 7:30am to 7:45am for scheduled maintenance.
