Healthy NY Monthly Data Submissions

To follow are instructions for the submission of monthly membership activity data for health plans with Healthy NY business

All monthly Healthy NY membership data must be submitted online via the DFS Portal. 

DFS Portal and DFS ID

Those conducting business on behalf of a regulated entity must use DFS ID to access the DFS Portal.

Users must receive an invitation to DFS ID from DFS or an Entity Administrator. You will use DFS ID credentials and multi-factor authentication (MFA) to log in, so be sure to have or download an authenticator app (e.g., Google or Microsoft Authenticator) to an available mobile device before getting started.

Due Date

The monthly submission is due by 12:00pm 15 days after the close of the month. For example, the monthly membership form that summarizes Healthy New York activity from 7/1/26 through 8/1/26 is due no later than August 15, 2026.

All health plans with approved Healthy New York forms must have an account in the Department's Portal. If there is no enrollment, email Brian Schechter at [email protected] each month by the 15th to report that there is no enrollment.

An authorized representative must log into the DFS Portal each month and submit the required Healthy New York data for each county by either completing the form for each county or by making an bulk submission. (Specifications for a tab delimited text file or an xml dtd [document type definition] file are available upon request.)

We recommend a monitor display resolution of 1024 x 768.

The ending totals for each county and each category should equal the starting totals for the next month's form. When completing the form, the in-force data for the beginning date will automatically be populated for each county based on the previous month's submission.

Grand total enrollment for all counties will be calculated for you in the form. Check these totals before submitting the form.

If the health plan finds after a monthly submission that the information submitted was incorrect, either because of retroactivity or other reasons, it is the health plan's responsibility to log on to the Department's web site and make a full resubmission.  Use the "New Submission" button, repeating the filing date.
