Fingerprinting: Officers/Directors of Insurance Companies

Fingerprinting is required for all officers/directors of insurance companies in connection with background investigations conducted by DFS. Officers/directors who have any address in New York State must be electronically fingerprinted.

The Department of Financial Services uses IdentoGO by IDEMIA for fingerprinting services. IdentoGO is the exclusive live scan fingerprinting provider for DFS as well as the New York Division of Criminal Justice Services, and is a certified FBI Channeling Agent. Only IdentoGO Centers located in New York State may be used, as this program operates under contractual agreements between DCJS and IdentoGO.

Applicants with an Address in New York State

Individuals with an address in New York State needing fingerprints taken must go to an IdentoGO center in New York State.

Appointment scheduling via the IdentoGO website is available 24/7/365. Appointment scheduling via the call center is available from 9:00 a.m. to 9:00 p.m. Monday through Saturday.

Fee

The applicant will be required to pay a $99.00 fingerprint processing fee at the time of the appointment. 

Receipt

The receipt issued by IDEMIA as verification of having been electronically fingerprinted must be attached to the applicant’s biographical affidavit and mailed to DFS.

When NAIC biographical affidavits for officers/directors are submitted to the Department, the receipts issued by IDEMIA as verification of having been electronically fingerprinted must be attached.

Card Scanned Fingerprinting

Fingerprint cards will not be accepted from an officer/director with any address in New York.

Any application bearing an address in New York State submitted with card scanned fingerprint cards will be rejected. Applicants who do not have any address in New York State and are unable to go to an IdentoGO by IDEMIA location in New York State MUST submit fingerprint cards.

Only fingerprint cards furnished to the applicant by the New York State Department of Financial Services can be used; out of state fingerprint cards are not acceptable and will be returned.

NOTE:  Only FBI Form FD-258 fingerprint cards may be used; any other type of fingerprint card will not be accepted and will be returned.

Pre-Enroll with IdentoGo

Officers/directors who do not have any address in New York State and are unable to go to a IDEMIA electronic fingerprinting location in New York are required to submit paper fingerprint cards to this Department along with NAIC biographical affidavits.

Applicants who do not have any address in New York State and are unable to go to an IdentoGO by IDEMIA location in New York State who must submit fingerprint cards MUST PRE-ENROLL on the the IdentoGO website and selecting the option to “Submit a Fingerprint Card by Mail”.

You will receive a pre-enrollment confirmation page, which includes a barcode printed on the top right of the page. 

After pre-enrollment, you must print out, complete and sign the pre-enrollment confirmation page.  This pre-enrollment confirmation page with the original signatures (no photo copies) must be attached to the paper fingerprint cards (two) and biographical affidavit and mailed to DFS with your licensing fee.

DFS Mailing Address

After registration the pre-enrollment confirmation page MUST be completed, printed, signed and the form with the original signatures (no photo copies) must be attached to the paper fingerprint cards and submitted to DFS along with the required NAIC biographical affidavit.

Mail the signed pre-enrollment confirmation page and the completed fingerprint cards to this Department with the application packet and licensing fee.
