2019-20 Annual Statements and New York Supplement Filing Checklists, Instructions and Forms
Per Circular Letter 15 (1999), Annual Statement instructions and blank forms will not be mailed to licensed companies by New York State. A complete set of general and detailed specific instructions, together with blank forms and other related supplemental/addendum forms for the New York Supplement and New York Statements, will be posted on the Department's website in January 2020. Download and print capabilities will be available.
