As per the instructions, report submissions are required to be made through the DFS Portal.

Complete the Survey

Download the MS Excel worksheet file “REPORT.xlsx” (link above), and complete the data required.

Save the worksheet as “REPORT.xlsx” in the original Excel file format; do not convert the file to a different version of Excel or "PDF".

After completing and saving the file, log in to the DFS Portal to upload the completed file (see below).

DFS Portal and DFS ID

Those conducting business on behalf of a regulated entity must use DFS ID to access the DFS Portal.

Users must receive an invitation to DFS ID from their Entity Administrator. You will use DFS ID credentials and multi-factor authentication (MFA) to log in, so be sure to have or download an authenticator app (e.g., Google or Microsoft Authenticator) to an available mobile device before getting started. To get started, use the DFS Portal button below and sign in using your DFS ID credentials:

NOTE: A portal session times out if there has been no activity for 30 minutes. Data entered into the portal will NOT be saved once the session has timed out, so be prepared and start a new session just before you begin entering data.

Once signed in, use the "Homeowners Insurance – Regulation 154 Quarterly Reporting" application to submit the filing.

IMPORTANT: This report file MUST be submitted through the DFS Portal application. DO NOT submit the report through other apps, such as the “Public Automobile Classifications (NYIL §308)” or “Insurance Company and Fraternal Benefit Society Filings” portal - it will not be processed and the company may be considered out of compliance with this statutory requirement.

Detailed Instructions

Use the "Homeowners Insurance – Regulation 154 Quarterly Reporting” application in “My Apps” and submit your report file (once completed per the instructions) as follows:

  1. In “Reporting Period” Choose the specific quarter/year for the report being submitted.
  2. For “Company included in this filing” choose the company you are making the submission for (for companies in a group, or with multiple programs, separate submissions must be made).
  3. Select the “Upload Document” button and select the file to be uploaded.
  4. Check the affirmation statement check-box prior to submitting the file.
  5. Select  the “Submit” button.
  6. You should receive an email confirming receipt of the report.
