HAZARDOUS MATERIAL REPORT FORMS

SECTION 168-i OF THE INSURANCE LAW

SECTION 209-u OF THE GENERAL MUNICIPAL LAW

In accordance with the provision of Section 168-i of the Insurance Law which was enacted under Chapter 870 of the laws of 1980, each insurer providing coverage against the peril of fire to commercial risks in this state shall be required to furnish its policyholders annually with a suitable supply of forms for reporting the presence of hazardous materials as required by Section 209-u of the General Municipal Law.

A supply of forms, including instructions, is available from:

The Office of Fire Prevention and Control has requested that orders for such forms be sent to it prior to December 1, 1980 stating the estimated number needed for the next 6 months. The order shall include a mailing sticker with the name and address of the company, and the name of a responsible person to whom the forms shall be delivered.
